 ge and a set of tools for learners to support planning with outcomes\, dat
 a-driven decision making\, and assessment within their organization. Non-p
 rofit organizations face increasing accountability to &ldquo\;show their i
 mpact&rdquo\; to governments\, donors\, and stakeholders. To respond to hi
 gher standards\, the vision for this course is for service providers to ha
 ve staff with the know-how to complete data collection\, as related to ser
 vice provision and the&nbsp\;<em>Creating Excellence Together</em>&nbsp\;a
 ccreditation standards.</p>\n\n<p>Learners will learn practical and intent
 ional approaches to setting and achieving goals. These approaches can and 
 will be applied in both personal and professional contexts to link theory 
 and practice.</p>\n\n<p>This course references&nbsp\;<em>Creating Excellen
 ce Together</em>&nbsp\;Standards to make content relevant to the work of c
 ommunity disability services. These standards receive specific attention b
 ecause they set the expectations for decision-making and evaluating outcom
 es for service providers. As well\,&nbsp\;this program includes individual
  reflection and group discussion.</p>\n\n<p>The course concludes with a te
 st\, consisting of 20 items. Learners must earn a score of 80% to earn a c
 ertificate of completion.</p>\n\n<p>By completing this course\, learners w
 ill be able to:</p>\n\n<ol>\n<li>Describe the&nbsp\;<em>why</em>\,&nbsp\;<
 em>how</em>\, and&nbsp\;<em>what</em>&nbsp\;of personal and organizational
  efforts\;</li>\n<li>Describe the personal vision for one&rsquo\;s life\, 
 and how this relates to outcomes\;</li>\n<li>Describe the components of pe
 rsonal well-being\;</li>\n<li>Create SMART Goals related to one&rsquo\;s w
 ell-being\;</li>\n<li>Differentiate between outputs and outcomes\;</li>\n<
 li>Define a logic model for the achievement of goals\;</li>\n<li>Define&nb
 sp\;<em>measurement</em>\,&nbsp\;<em>assessment</em>\, and&nbsp\;<em>evalu
 ation</em>&nbsp\;and explain how these concepts support the achievement of
  goals\;</li>\n<li>Relate measurement and goal-tracking to an Individual S
